Accessible Information Standard

Tell us about your communication needs.

The practice will always try to provide information and correspondence in a format that patients will find easy to understand. If you have a communication problem of any kind please let us know and we will work with you to make sure we provide information in the way that you would like us to. If you need the practice to provide information in a different format such as large print or easy read please let us know and we will arrange this.

You can download our Accessible Information Form here – please complete and bring to the surgery the next time you attend to make us aware of your communication requirements.

Please view our Accessible Info Poster.
